To All Argo Student Athletes

In preparation for the upcoming athletic seasons, the sports medicine department needs your current information (medical, insurance, emergency contacts, and contact info). Everything you need to provide is included in the links listed below. Please take the time to review and fill out the necessary paperwork.

Prior to participating on any athletic team for the University of Providence, all student athletes must provide the Athletic Department with all required information asked for by the Sports Medicine staff.  To expedite this process, the University of Providence uses an online data entry system called SportWareOnline.  Please find directions below.

All New/Transfer student athletes and any Returning student athletes going into their 3rd year at the university are required to have a signed physical by a (N.P., P.A., or M.D.) and a completed history form to be cleared to participate.  Please find Preparticipation Physical and History Form below.

All of the paperwork must turned into the athletic trainers before you may participate in any team oriented practices or competitions which includes pre-season.
